Breast Lift Surgery in Miami

Body image is an important part of feeling good, desirable, and normal. Most women compare themselves to a cultural idea of beauty. Commonly, women with sagging breasts and breasts with loose, stretched skin are dissatisfied with their physical appearance. These changes can be a result of genetics, pregnancy, weight loss, and aging or any combination thereof.

An ideal candidate for a breast lift procedure, or mastopexy, is someone who is uncomfortable with the amount of loose skin and sagging of her breasts. An ideal breast lift patient is also free of any complicating pre-existing medical conditions including heart disease, hypertension, diabetes, skin/connective tissue disorders, etc.

Breast Lift Consultation

The initial consultation is extremely important to review the patient’s medical history, goals, and expectations, as well as build mutual trust and rapport. Numerous breast lift before and after photos are reviewed at our office in Miami, Florida. The breast lift surgical procedure is carefully explained and the alternatives are discussed. Understand that each patient has different breast contours and sizes and different expectations and requests. No one procedure can be expected to fulfill each individual’s needs.

A complete evaluation reviewing expectations and physical examination is needed to determine the best choice of procedure(s) for each individual patient. The breast lift surgical plan, as well as the incision/scar placement, is reviewed.

It is also important to understand that breasts are naturally asymmetrical. Every effort is made to make the breasts symmetrical and as close as possible to the desired size. However, no guarantees as to the exact size, shape, or symmetry can be made by any plastic surgeon.

Prospective patients are encouraged to speak with previous patients about their breast lift surgery and result while Dr. Gershenbaum and his staff are always available for questions. Following the initial consultation and after appropriate time for reflection and decision-making, an appointment is scheduled for routine preparation and informed consent. Standard Breast Lift Surgery Procedure

Miami breast lift surgeon Dr. Sam Gershenbaum uses a “keyhole” pattern or “anchor” shaped incision. The incisions made during breast lift surgery extend in three different directions:

  1. Entirely around the areola (the pigmented area around the nipple)
  2. From the areola in a vertical direction down the crease under the breast
  3. Horizontally along the fold or crease under the breast extending for a variable length

Generally, the greater the amount of lifting, the longer will be the horizontal portion of the scar. Excess skin is removed and sculpted from the inner and outer quadrants of the breast and from the area above the nipple. The remaining breast tissue and nipple are relocated in this keyhole pattern and the incisions closed.

Dr. Sam Gershenbaum does not use clips or staples to close the skin. All incisions are closed with sutures predominantly below the surface of the skin to avoid cross-hatching suture marks and to optimize the chances for the finest scars.

A conservative size breast implant or “auto” augmentation to preserve fullness in the upper portion of the breast may also be performed at our Miami center with the standard breast lift procedure. This additional procedure utilizes a portion of breast tissue left connected to the chest wall and its blood supply. This segment of tissue is then passed up and under a two-inch bridge of pectoralis muscle and secured.

Additional Breast Lift Surgery Techniques

Miami breast liftOur Miami breast lift center offers several techniques to lift and reshape the breast and reposition the nipple. The size of the breasts, the amount of loose skin and sagging, along with patient expectations will determine the extent of the mastopexy incisions. The size of the areola (the pigmented area around the nipple) would also be reduced during the procedure if desired. A minimal scar breast lift procedure encompasses several newer techniques aimed at reducing scars:

  • With a circumareolar-vertical or “lollipop” incision technique, the skin is elevated and the excess is removed from around and under the areola (the pigmented area around the nipple). The resulting scar extends completely around the areola and vertically down to the crease under the breast. A conservative size breast implant or the “auto” augmentation technique may also be performed with a breast lift to enhance the fullness in the upper portion of the breast.
  • With a circumareolar or “Benelli” type breast lift surgery (sometimes referred to as a “donut” mastopexy), the skin is elevated and the excess is removed from around the areola only. The resulting scar extends completely around the areola. Similarly, a small breast implant may be placed to enhance the fullness in the upper portion of the breast.

A standard breast augmentation, with saline or silicone implants may be performed with any of these breast lift procedures to increase the overall cup size of the breasts. Following breast lift surgery, a light dressing, a support bra and/or ace bandage are applied.

Breast Lift Recovery

Following breast lift surgery by Miami surgeon Dr. Sam Gershenbaum, patients generally report mild to moderate discomfort, which is easily controlled with medications. This will vary somewhat depending on the extent of surgery and procedure performed.

When breast implants are used in conjunction with a breast lift procedure, increased swelling and fullness in the upper part of the breast or chest area and in the midline between the breasts may be noted. Varying degrees of swelling, bruising, and firmness subside over several weeks to months.

The circumareolar-vertical and circumareolar breast lift techniques often result in varying degrees of “pleats” or folds around the incision, which will resolve over time. Sutures (stitches) are removed after seven days. Patients can generally return to work after several days to one week. Light exercise may begin after four to six weeks. A support bra is generally worn for two to three months following breast lift surgery and is encouraged as much as possible thereafter to maintain the contour.

Following breast lift surgery, recovery, and healing will vary from patient to patient and depend on the extent of the surgery. It must also be remembered that complete healing takes time and patience, taking up to one year or longer before realizing the final results of the surgery. It can take this amount of time for scars to soften, lighten, and fade as well as for the breast tissue to soften, relax, and remodel itself. If implants were used, they too will settle over many months to come.

The breasts will get slightly smaller and descend to a certain extent as swelling subsides and the effects of gravity and tissue relaxation act on the newly tightened skin envelope. Most patients find this beneficial in that the breasts look more natural over time. The ability to breast-feed after a breast lift (mastopexy) is generally preserved, but cannot be guaranteed.

Complications Associated with a Breast Lift

As with most cosmetic surgical procedures, complications are unlikely when performed by a skilled and competent surgeon. Complications can include, but are not limited to: anesthesia risks, bleeding, infection, tissue loss, delayed wound healing with obvious wide and/or raised scars, numbness, altered nipple sensation, asymmetry in breasts and/or areolas, enlarged areolas, and need for revisional surgery. If breast implants are used, all the potential risks/complications for breast augmentation will also apply.

Also rare, but possible, are blood clots or emboli (blood clots which dislodge and travel in the blood stream to distant sites), which can be fatal.

At the Aventura Center for Cosmetic Surgery, Venodyne® calf compression devices are used for every breast lift surgery (as well as other surgeries). A Venodyne® is a pressure cuff that fits over each lower leg from the ankle to the knee, circulating every one to two minutes to reduce the pooling of blood in the lower extremities and further reduce the possibility of blood clots or emboli.

Smokers must stop smoking well in advance of surgery. Smoking seriously decreases blood circulation in the skin, which increase the risks of complications and poor healing.

Breast Lift Surgery in Miami, Florida
Breast lift surgical procedures are performed under general anesthesia at our state-of-art outpatient facility just north of Miami, Florida. Local anesthesia with intravenous sedation may also be used in some circumstances. Depending on which breast lift technique is used, the surgical procedure could take anywhere from two to five hours. Prior to breast lift surgery, the lines of incisions, the new nipple location, and remainder of the surgical plan is outlined on the breasts. After speaking with the anesthesiologist, the patient is brought to the operating room. Once the patient is asleep, local nerve blocks and a bleeding reduction medication are administered.

Breast Changes Are Common

As time passes, women commonly experience changes in the way their breasts look and feel. These changes frequently include a loss of skin elasticity and reduced firmness, both of which cause the breasts to droop. The Limitations of a Mastopexy

A breast lift cannot significantly increase or decrease the size of the breast, or round out the upper portion of the breast. Women who would like their breasts to look fuller may want to ask Dr. G about having a breast lift in conjunction with a breast augmentation. Women who would like to reduce the size of their breasts can combine a Mastopexy with a breast reduction.

Who Should Consider a Breast Lift in Miami?

Women who are unhappy with the way their breasts look and/or feel may want to consider a breast lift.

A good candidate for a breast lift with Dr. Gershenbaum is:

  • Physically healthy
  • A nonsmoker
  • Able to maintain a stable weight

Issues that a breast lift with Dr. G in Miami can address:

  • Flat, elongated breasts
  • Pendulous breasts (sagging, droopy breasts)
  • Asymmetrical breasts (one breast falls lower than the other)
  • Stretched, loose skin

An Initial Consultation for a Breast Lift with Dr. G in Miami

In order to receive optimal breast lift results, patients need to be candid with Dr. Gershenbaum and his staff.

When women meet with Dr. G for an initial consultation, they need to provide him with information related to:

  • The medications they are currently taking as well as herbal supplements and vitamins
  • Medical conditions (i.e., high blood pressure, diabetes, heart disease, etc.)
  • Drug/Latex allergies
  • Previous surgical procedures and/or medical treatments
  • Lifestyle choices – tobacco use, alcohol consumption and/or recreational drug use
  • Family medical history, especially information related to breast cancer
  • Mammogram results and/or results of previous breast biopsies

At this point in time, Dr. Gershenbaum may:

  • Evaluate the patient’s general health
  • Inquire about pre-existing health conditions
  • Discuss risk factors related to a breast lift procedure
  • Examine the patient’s breasts, take measurements of their shape and size
  • He will note the placement of the patient’s nipples and areolas in addition to the quality of her skin
  • Dr. G may want to take pictures to place in her file

Once he has completed examining the patient’s breasts, he will discuss her options and recommend a course of treatment. In addition, Dr. Gershenbaum will answer any questions the patient has, he will address likely outcomes, potential complications or risks as well as discuss the type of anesthesia he believes will serve the patient the best during her breast lift surgery.

Patients are Encouraged to Ask Questions

Dr. Gershenbaum wants his patients to understand every aspect of the breast lift procedure. Additionally, Dr. G recommends that his patients who are having a Mastopexy in conjunction with a breast augmentation or breast reduction understand how those procedures are performed as well.

Many patients feel anxious prior to their breast lift, this is natural and patients should not be shy about mentioning these feelings to Dr. G. He encourages all of his patients to ask questions during their initial consultation.

Preparing for a Mastopexy with Dr. Gershenbaum in Miami

Once Dr. G determines that a patient is a good candidate for a breast lift, he may request that she:

  • Visit her primary care physician to have a medical evaluation.
  • Have lab tests performed (including blood work).
  • Adjust her medications prior to her breast lift.
  • Take certain medications before her procedure.
  • Have a mammogram before her Mastopexy to serve as a ‘baseline’ mammogram. Following her breast lift, Dr. G will most likely request that she has another mammogram. These mammograms will be used to detect future changes in her breast tissue.
  • Avoid taking certain herbal supplements and medications that are known to increase bleeding, including nonsteroidal anti-inflammatory drugs (NSAIDs).
  • Stop smoking for at least four weeks before her breast lift surgery and four weeks after. This is very important because nicotine affects the ability of the body to heal itself. Women who continue smoking directly before and directly after their Mastopexy tend to have more complications than women who do not smoke at these times.

Preparing for a Breast Lift in Miami

Prior to their breast lift, patients should either purchase already made meals or create meals that just need to be warmed. Ideally, Dr. G would like his patients to have someone they trust stay with them for several days following their procedure.

Patients Need to Create Themselves a Recovery Area at Home

Patients need to create themselves an at-home recovery area. Since patients must avoid lying flat for the first few days after their Mastopexy, many of Dr. Gershenbaum’s patients choose to create a recovery area in their living room, especially if they have a recliner.

Once created, this recovery area should have everything the patient will need for the first few days after her breast lift. TV trays are perfect for this task because they can be placed on either side of the recliner.

Creating a Recovery Area – What Patients Need

Patients need to have extra pillows and throws nearby.

In addition, patients need to have the following items on TV trays within easy reach:

  • Remote controls and extra batteries
  • A cell phone and the cell phone’s charger (to avoid the need to bend, charger should already be plugged in and placed on the TV tray)
  • Baby wipes and facial tissue
  • Lip balm
  • A cooler with ice packs, loose ice (for beverages) and snacks that need to be kept cool
  • Beverages
  • Healthy snacks including granola bars, crackers, fresh vegetables and fruits that are ready-to-eat, hard-boiled eggs, etc.
  • 2 Trash cans (one for garbage and the other in case of nausea that leads to vomiting)
  • Medications
  • Disposable cups, napkins, silverware and plates

A Breast Lift in Miami – The Procedure

A breast lift is an outpatient procedure. Patients will receive medications to keep them comfortable during their surgery. For this reason, patients are not permitted to drive and need to bring a driver with them to Dr. G’s Surgery Center on the day of their procedure.

1. Anesthesia is Administered

An anesthesiologist will administer medications (IV sedation or general anesthesia) that will keep the patient asleep throughout her surgical procedure. The type of medication used will be discussed with the patient prior to her Mastopexy.

2. The Incision is Created

Once the patient is asleep, Dr. Gershenbaum makes the incisions.

The incision technique he uses depends on:

  • The quality of the patient’s skin
  • The position and size of her areolas
  • The amount of extra skin the patient has
  • The shape and size of the patient’s breasts
  • The elasticity of the skin on the breasts

Incision Techniques for a Mastopexy with Dr. Gershenbaum in Miami

Incision patterns that are commonly used during a breast lift are:

The Lollipop Incision

An incision is created around the patient’s areola and then vertically down to her breast crease.

The Anchor Incision

An incision is created around the areola, vertically down to the breast crease and then horizontally along the breast crease itself.

The Donut Incision

An incision is created around the areola.

The Keyhole Incision

An incision is made around the areola and then two vertical incisions are made down toward the breast crease.

3. Dr. G Reshapes the Breasts

To improve the firmness and contour of the patient’s breast, Dr. Gershenbaum lifts and shapes the underlying breast tissue. He repositions the areola and nipple, moving them to a more youthful height. He may also excise the extra skin around the areola’s perimeter and remove any unnecessary, overstretched skin on the breast.

4. The Incisions Are Closed

Once Dr. G has reshaped the breasts and removed the excess skin, he tightens the skin and closes the incisions. Some of the incision lines are concealed within the natural contours of the breast.

Dr. Gershenbaum does not use staples or clips; instead, he uses sutures, most of which are placed below the surface of the skin. This optimizes the possibility for the finest scars.

Sutures are Layered

By layering the sutures within the breast tissue, the sutures provide support for the newly shaped breasts. Any suture marks that are visible will usually fade with time.

5. A Drainage Tube May be Placed

Dr. Gershenbaum may temporarily place a small, thin tube beneath the skin. This tube is used to drain excess fluid or blood away from the treated area, which helps reduce inflammation.

6. Dressings and a Support Bra Are Applied

Once the incisions are closed and the drainage tube is in place, Dr. G will apply a light dressing and support bra over the patient’s breasts. The support bra helps maintain the new shape of the breasts.

7. Initial Recovery

Directly following a Mastopexy, patients remain in the recovery area until Dr. G releases them to their driver.

Before leaving the recovery area, patients receive information related to caring for their breasts, medication usage, when to follow up with Dr. G, what to watch for in relation to the incision site and her general health as well as her physical restrictions.

The Results

The results of a breast lift are visible immediately. If Dr. G performs a breast augmentation in conjunction with a breast lift, patients should expect to experience increased fullness and swelling in the upper portion of the breast, chest and midline area. Patients will have varying degrees of bruising, swelling and firmness, all of which should subside within a few weeks to several months.

Circumareolar and circumareolar-vertical breast lift techniques frequently result in folds (pleats) around the incision. However, these pleats will resolve as time passes.

Dr. Gershenbaum removes the stitches seven days after surgery and most patients can return to work within a week.

After four to six weeks, patients are usually allowed to begin a light exercise routine.

To help maintain the contour of their breasts, patients receive a support bra. Directly following a Mastopexy, this bra is usually worn for several months. Once the mandatory several months have passed, Dr. G encourages his patients to continue wearing the support bra periodically to help maintain the new contour of their breasts.

The final results of a Mastopexy are not visible for a few months. Results attained through a breast lift are long lasting and although incision lines are permanent, they do fade.

Some Patients May Have an Auto Augmentation During Their Mastopexy

To preserve the fullness of the upper portion of the patient’s breast, a conservative size breast implant may be placed during a Mastopexy. This procedure uses a portion of the patient’s breast tissue that is still connected to the chest wall and the patient’s blood supply to add fullness to the upper portion of the breast. This breast tissue is secured under the pectoralis muscle.
