Butt Implants (Gluteoplasty)
Many men and women are dissatisfied with the size, shape and contour of their buttocks. Underprojected, flat and/or sagging buttocks do not fit their ideal self-image. Body-image is an important part of feeling good, desirable and confident. For many, this surgical procedure can dramatically affect the quest for a beautiful body.
Buttock implants, also known as gluteal augmentation, gluteal implants or gluteoplasty, is a surgical procedure designed to add fullness and projection to the underprojected or flat buttock area.
The definition and contour of the buttocks is due to the size and shape of the surrounding tissues. Excess and localized fat deposits of the hips, waist, lower back and upper thighs may obscure the definition and shapely contour of the buttocks. Frequently, liposculpture of any combination of the hips, waist, low back and upper thighs is done simultaneously with buttock implants to enhance the shape and contour of the buttocks.
For those who have excess sagging of the skin or folds of the buttocks, a buttock lift from above, or a local skin excision from the crease below, may be desirable. This is usually reserved for more extreme cases with the understanding that more extensive scarring will result. Generally, these scars are easily hidden within the bikini, bathing suit or undergarment lines.
An ideal candidate for buttock implants (gluteoplasty) is a man or woman dissatisfied with the underprojection or flat appearance of their buttocks who is free of any complicating pre-existing medical conditions including heart disease, high blood pressure, diabetes, skin/connective tissue disorders, etc.
Butt Implant Consultation
The initial consultation is important to review the patient’s medical history, goals and expectations as well as build mutual trust and rapport. Before and after photos are reviewed, the procedure is carefully explained and the alternatives discussed. Each patient has different buttock/gluteal contours, projection and sizes, as well as different expectations and requests. No single procedure can fulfill every individual’s needs. A complete evaluation and physical examination is performed for each individual patient to ensure that expectations can be met as well as to determine the best choice of procedure(s) and the size and shape of the implants to be used. The surgical plan and the incision/scar placement are reviewed. Every effort is made to accomplish the desired set of goals, however, no surgeon can guarantee exact size, projection and contour of the buttocks.
Prospective patients are encouraged to speak with previous patients about their surgery and results. Dr. Gershenbaum and his staff are always available for questions. Following the initial consultation, and after appropriate time for reflection, an appointment is scheduled for routine preparation and informed consent. Additional questions and concerns may also be addressed. Routine blood analysis and medical clearance is required before surgery.
This procedure involves inserting a soft, high grade, solid silicone, implant into the muscular space of the buttock area. To the touch, it has the feeling and consistency of a flexed muscle. Unlike breast augmentation, after this procedure is properly completed and healed, there is no reason to require additional surgery. These implants can be round or oval and come in a variety of sizes. A consultation and physical exam is needed to determine the appropriate size and shape for individuals.
The butt implant procedure is performed in our state-of-the-art out patient facility in Miami under general anesthesia. Epidural (spinal) anesthesia may also be used at times. This surgical procedure takes approximately one and one-half to two hours to perform. Liposculpture and/or lifting procedures can add an additional one to three hours or longer. The surgical plan, including implant placement and liposculpture, is outlined prior to surgery. While in the sitting position, it must be assured that the implant is positioned above the weight-bearing area. After speaking with the anesthetist/anesthesiologist, the patient is brought to the operating room. After asleep, local nerve blocks, combined with a medication that shrinks blood vessels and capillaries to reduce any bleeding, are instilled.
A 5 to 7 cm incision is made vertically in the midline between the upper aspect of the buttocks. A “pocket” or space is created under each gluteal muscle of the buttocks corresponding to the plan outlined on the skin. The implants are then inserted. Prior to the closure, a long acting anesthetic is poured around the implants, which dramatically reduces post-operative discomfort. The incision is closed with sutures below the level of the skin to avoid cross hatching and suture marks.
At that time, Liposculpture, if planned, is then performed corresponding to the areas outlined on the skin, for further enhancement and definition.
A buttock-thigh lift is reserved for more extreme cases of excessive drooping of the buttocks and can be performed in conjunction with buttock (gluteal) implants. Other times, for excessive sagging along the buttock folds, simple local skin excision can be performed with the incision/scar following the crease below. These two additional procedures are reserved for more extreme cases, with the understanding that more extensive scarring will result.
At the conclusion of surgery a light dressing and an elastic compression garment is applied over the surgical areas.
Butt Implant Recovery
After surgery, patients will experience some swelling and soreness/pain around the surgical area. The uncomfortable part of recovery lasts seven to ten days, and pain is controlled with oral medications. Gentle stretching exercises with knee raises begin immediately after surgery. The patient will remain on their stomach and sides for the first 48 hours.
Compression garments are worn for two to three weeks. Routine activities may resume at two weeks and exercise started at 8 weeks post-op. Recovery and healing will vary from patient to patient and depends on the extent of surgery. Complete healing takes time and patience, taking several months before appreciating the final results.
Complications in Buttock Implant surgery
As with most cosmetic surgical procedures, complications are unlikely when performed by a skilled and competent surgeon. Complications include, but are not limited to:
- Anesthesia risks
- Fluid accumulation under the skin (which may need to be drained)
- Nerve damage
- Poor wound healing
- Unsatisfactory scars
Also possible is asymmetry, displacement, hardening of scar tissue around the implant and need for revisional surgery (or even removal of the implants). Also rare, but possible, are blood clots or emboli (blood clots which dislodge and travel in the blood stream to distant sites) which can be fatal.

Venodynes are pressure cuffs that fit over each lower leg, from the ankle to the knee, circulating every one to two minutes to reduce the pooling of blood in the lower extremities and further reduce the possibility of blood clots or emboli. In addition, smokers must stop smoking well in advance of surgery. Smoking seriously decreases blood circulation in the skin, which increases the risk of complications and poor healing.

Creating the Surgical Plan

After the initial consultation, patients should take some time to reflect. Once a patient confirms that he or she is interested in a buttock augmentation, a pre-surgical appointment is made. At this appointment, Dr. Gershenbaum will address any questions or concerns the patient has. Preparing for a Buttock Augmentation with Butt Implants in Miami
Create a Recovery Center
Patients should create themselves an area at home where they can recover: Since they will need to avoid putting pressure on their buttocks for 48 hours following their procedure, most patients create their recovery area in the living room where they can lie on their stomach or side on the couch. Patients need to have a variety of things within easy reach as they recover. TV trays are useful for this purpose and can be placed at the end of the couch for easy access.
Here are some of the items that need to be readily accessible:
- Snacks (i.e., fruit, vegetables, granola bars, etc.)
- A cooler with ice packs and loose ice for beverages
- A cell phone with its charger or a phone connected to a land line
- Remote controls for the television, satellite/cable (extra batteries)
- Facial tissues
- Baby wipes
- A trash can
- Disposable plates, cups and silverware
Stop Smoking and Avoid Secondhand Smoke
Patients must discontinue smoking for at least 4 weeks before and 4 weeks after their buttock augmentation with butt implants: This includes being exposed to secondhand smoke. Avoiding secondhand smoke and discontinuing to smoke is very important because smoking negatively affects the body’s ability to heal itself.
Discontinue Taking Medications on Dr. Gershenbaum’s List
Certain medications, vitamins and herbal supplements must be discontinued for at least 2 weeks before surgery. Dr. G provides a list of these products on his website.
Ask a Trusted Friend or Family Member to Stay Overnight
Dr. G recommends that his patients ask someone that they trust to stay with them for at least the night after their procedure.
Gluteoplasty: The Procedure
During a gluteoplasty, patients receive anesthesia; therefore, patients are not permitted to drive themselves home following their surgery.
On the day of their procedure, patients need to arrive at our outpatient facility with their driver. A butt implant surgery can take anywhere from one to two hours. When additional procedures are performed (i.e., a butt lift or Liposculpture), that amount of time can increase by one to three hours, depending.
Prior to surgery, Dr. G discusses his surgical plan with the patient. Issues discussed include implant placement and which areas of the body he can improve with Liposculpture.
1. Pre-Surgical Marking
Prior to surgery, the skin on the patient’s buttock is marked according to Dr. G’s specific surgical plan. These markings essentially provide Dr. G with an ‘outline’ to follow when creating the incisions for placing the implants and while performing Liposculpture.
Patients receive general anesthesia, local nerve blocks and medication designed to shrink the capillaries and blood vessels during their buttock augmentation. By shrinking the capillaries and blood vessels, the incidence of bleeding is significantly reduced.
Dr. G has an anesthesiologist/anesthetist administer the general anesthesia. The anesthesia ensures that the patient remains asleep throughout the butt implant surgery.
3. The Incision
Once the patient is asleep, Dr. Gershenbaum makes a vertical incision (5 to 7 cm) in the midline between the upper part of the patient’s buttocks.
4. The Pockets Are Created
Dr. G creates a space under each of the patient’s gluteal muscles. These spaces or “pockets” will house the butt implants.
5. The Implants Are Inserted
Once the pockets are created, Dr. Gershenbaum gently inserts the implants.
6. A Long-Acting Anesthetic is Poured Around the Implants
Prior to closing the incisions, patients receive a long-acting anesthetic. This anesthetic is poured around the butt implants and drastically reduces the amount of post-operative discomfort patients experience.
7. The Incisions Are Closed
Once the long-acting anesthetic is applied, the incisions are closed using sutures. These sutures are placed below the level of the skin; which avoids suture marks and cross hatching.
If Applicable, Liposculpture is Performed
If the patient is also having Liposculpture on other areas of the body that affect the way his or her buttock looks, Dr. G will perform that procedure according to the pre-surgical markings on the patient’s skin.
8. A Light Dressing and a Compression Garment Are Applied
Directly after the butt implant surgery, a light dressing and elastic compression garment are applied over the areas Dr. G treated.
9. Calf Compression Devices Are Used in Recovery Following Surgery
At Dr. G’s Cosmetic Surgery Center, Venodyne Calf Compression Devices are used. These devices fit over the lower legs, extending from the ankle to the knee. These compression devices reduce the amount of blood that pools in the lower extremities, which reduces the likelihood of blood clots and embolism.
Prior to leaving the outpatient surgery center, patients receive detailed information explaining how they should take care of their incisions, which activities they need to avoid, when their follow-up visit with Dr. Gershenbaum will be and signs to watch for that indicate infection.
Recovering from a buttock augmentation with butt implants:
- Directly following surgery, patients should expect to experience inflammation and pain/soreness around the surgical area.
- The uncomfortable part of recovering from a buttock augmentation with butt implants usually lasts from 7 to 10 days. Dr. Gershenbaum will recommend or prescribe medications to alleviate his patients’ pain.
- Immediately after surgery, patients will begin performing gentle stretching exercises with knee raises.
- Patients will need to remain on their sides and stomachs for at least 48 hours after their butt implant surgery.
- The compression garments that patients receive during recovery will be worn for 2 to 3 weeks.
- Two weeks after their surgical procedure, patients can return to their normal routine.
- At 8 weeks post-op, patients can begin exercising.
- It can take up to 6 months for the final results of a buttock augmentation with butt implants to become evident.
Recovery and healing times vary from one patient to the next. The speed of recovery depends greatly upon the extent of the surgery performed and the patient’s adherence to the restrictions placed following his or her butt implant surgery.
